Your account will be reviewed and assigned to a site before portal access is enabled.

First Name Last Name Assigned Username Password Confirm Password Passwords do not match. Email Phone Number Phone number must be in 123-456-7890 format.

Training Lieutenant and above will assign your site and team after the account is created.
